Your loan
What kind of loan is it? It’s on your statement or your closing papers.
“Not sure” is fine. Ask your mortgage company who owns or guarantees the loan, and which application form and document list apply. Its name alone does not tell you the loan type.
Is this a direct loan from U.S. Department of Agriculture, or a USDA-guaranteed loan through a mortgage company?
For a conventional loan, you can check the Fannie Mae lookup or Freddie Mac lookup. Enter any identifiers only on the official site. If neither identifies it, ask your mortgage company to confirm the owner.
Your income
Where does your money come from now? Monthly, before taxes.
This can count if they agree to share proof of their income and live there.
Your income
The monthly amounts you entered. The company verifies what counts for your program.
Your program’s paperwork
Form 710 applies to Fannie Mae and Freddie Mac preparation. For other loans, confirm the company or USDA servicing office’s form and document list for your review stage.
This shows how the rules work. It's not a decision. Your mortgage company or USDA servicing office makes the decision.

Your savings
About how much do you have right now? A rough number is fine.
401(k)s, IRAs and college savings plans are left out. Lenders don’t count them, and you don’t need to list them.
Form 710Your home
Do you live there? The federal rules that pause foreclosure while your mortgage company reviews you cover the home you live in.

These go on page 1 of the form. Skip anything you’d rather write by hand.
Leave the Social Security number line blank in this draft. If your mortgage company requires the last four digits, write them on the form by hand. HomeBase’s optional SSN field is only in full-service counselor intake and is sealed separately; that saved number is excluded from the application package.
Your loan number goes at the top of page 1. Copy it from your statement by hand.
